Apply for a Library Card

1. Upload your proof of residence and photo ID (Driver's License) through the form below.

Both steps must be complete to apply for a library card. Once you have uploaded your documents AND completed the application, a staff member will be in contact with you shortly. 

All submitted documents will be used to verify your information. After verification these documents will be destroyed for security. Please submit documents as a .jpg or PDF.

Library Card Policies

Bella Vista Public Library cards are available at no cost to all residents of Benton County, POA employees, City of Bella Vista employees, and Bella Vista business owners and their employees.
Out of county adult residents may still get a library card, for an annual $10 fee. There is no fee for out of county children.

To get a library card, you will need to bring the following items:

  • Government Photo ID (Driver's License)

  • Proof of Residence-Name and Address must be visible (utility bill,  letter from employer, rental manager or the equivalent. P.O. Boxes cannot be used to verify residential status.)

A parent or legal guardian must have a valid library card and their signature is required for anyone under the age of 18 to obtain a card. All resident and non-resident library cards expire 12 months from the issue date.
